The registration requires an immediate payment via paypal. Without the payment your form will still be submitted but your registration will be incomplete. Flag Football practices will start Thursday, September 4th at Mackle Park. More information will be emailed to all players once registration has been closed. If you register after 09/01, there is a fee for late registration. All participants MUST sign the Code of Conduct and Parental consents before participating in our programs. **PLEASE NOTE: If you are registering for an age group that says FULL, please do not pay via PayPal. Your child will be placed on a waitlist and you will be contacted if a spot opens up.
WAIVER I herby give my child permission to participate in Optimist Club of Marco Island (“OCMI”) sports programs and make the following claims to the terms below as related to his/her participation in the OCMI sports program. I also confirm that I agree to the term in the OCMI Proper Conduct Policy Agreement. USE OF NAME AND PHOTO I hereby agree to the following terms regarding my child’s name or photograph as published on any media including but not limited to websites, newspapers, newsletters or any other publicly assessable media. I also to agree to allow my child’s name and/or photo to be published on the password protected areas of the OCMI website and its related portals. I allow for the publication of my child’s name and/or photo on any publicly assessable media (i.e., Marco Island newspapers, OCMI website, etc) for purposes of promoting the OCMI programs to local residents. MEDICAL RELEASE I hereby give permission for any and all medical attention to be administered to my child in the event of accident, injury, sickness, etc., under the direction of a Optimist Club of Marco Island representative to include but not limited to coaches, coach’s assistants, board members or league committee members, until such time as I may be contacted. I understand that I am responsible for accident insurance coverage for my child. I also assume the responsibility for the payment of any such treatment. INDEMNIFICATION I do hereby release, absolve, indemnify, and hold harmless the Optimist Club of Marco Island, Inc., and all persons engaged in any manner in the OCMI sports programs from any and all claims and from any actions or negligence of the Optimist Club of Marco Island, its members or agents. This agreement is effective through the period ending May 31, 2026 I understand and agree with all the terms of this disclosure and verify that fact with typing my full name above.
